What size is the SD card? I had many of the same problems when I loaded a 64 Gb SanDisk full of music and video. I spent a week or so trying different things eg making sure that the card was formatted to FAT32, flashing the firmware about 20 times because I think it corrupted my firmware.
In the end, I switched to a 32 Gb card.

THIS seems to be what caused the problem in my case too.
I also have this SanDisk 64 GB microSD card and now I'm guessing that is what is causing the problem. I will try to format it in my Xperia Z and then use it. Hope that helps. Otherwise I will have to move to a smaller drive. The 64 GB was working fine with Andriod 4.2.2 though. I just want to move back to the older 4.2.2.
Ok, I'm now running the Xperia Z on Lollipop and still with the SanDisk 64 GB microSD card plugged in. But evey time I upgrade the OS, I empty the microSD card and reformat it from Settings > Storage > Erase SD Card.
