This is most noticeable on the Lollipop settings dropdown (the menu you get when you swipe down from the top of the screen twice or with two fingers) but also occurs when I have many notifications.
All the icons on the lower two rows (Flashlight, Aeroplane Mode, Portrait, Hotspot, Mobile data & Cast screen) nearly always do nothing when I tap directly on them. It is as if the touch-area of the icon is very small, so 95% of the time I don't manage to trigger the action. I have found that I can trigger the icons on the 2nd row by tapping slightly above them, though it's still hit and miss.
Similarly if I have many notifications I struggle to swipe away the lower ones and pressing 'clear all' is very difficult.
I can post a video showing touches if helpful.
Edit: Turning the device off and on again fixed it, at least temporariliy. Will post a vid if it happens again - it's a really annoying bug when it happens!

That's odd, perhaps it's a hardware issue, lets test it
settings > about phone > Diagnostics > test > touchscreen.
@uliwooly - already did that, it's fine. I've only noticed touches not registering on the dropdown settings & notifications. Does no-one else have this?

@ulwoo Yeah ok a reboot (not even in safe mode) fixed it - you should probably advise people to try that before a safe mode reboot btw! I have edited my original post to say a reboot fixed it.
Still a weird one though, wonder how I got into that state? It was completely reproducible, for hours/days, until the reboot.
