Volume just goes up when it's down low

Hi, I have been having this problem for a while now. Everytime I Al listening to music or on YouTube and I have my volume down low. It just shoots all the way up! Even when I have my earphones in and out. It collude be 2 minutes into the don't or 3 songs later it's very random but very annoying.

Is there any way to fix this? ☺

This might be an app acting up. Have you checked if the same occurs in safe mode?
