To turn on the loudspeaker during a call, press the icon looking like a speaker (Audio output).
It sounds to me like your proximity switch might be acting up. You can test this under Settings > About phone > Diagnostics > Test > Ear proximity. Did the phone start vibration directly when you started the test?
