Mystery app icon.

Hi all, I noticed today that there is a dark app icon in my app list. There is no name to it, and i can't delete it. Any ideas as to what it is?

what happen if you tap on it?

Nothing at all. Press and hold, and it says add to home screen, but when I do, it disappears, from the home screen.

@Mstang@Slawovirus1 have you guys tried a restart? have you uninstalled some app lately?
