I just bought my xperia P a couple of days ago and have noticed that all my appointments sync properly, except for birthdays.
The birthdays are always one day off.
Strangely my girlfriends Xperia U does NOT have this bug, so it can't be ICS related.
This also isn't the typical Android bug since the series never go before 1970.

So, found the Solution myself.
First of all, for all future references:
I have Android 4 ICS and am running an Exchange 2010 Server.
Symptoms: All occurances that STARTED prior to 1980 were split over two days. Normaly with a start time of 10 or 11 PM on the day it starts (The persons Birthday for example) and ending 24 hours later, normaly the day AFTER the Birthday/occurance.
Solution: Edit the Occurance so that it STARTS after 1980. Problem solved.
Hmm, seems strange. So these birthdays are added in your calendar on the exchange server and not on a for example Google calendar?
exactly. My Email, Contacts, and Kalender are stored on an Exchange 2010 Server

It is not a solution to change the birthday. If you add a birthday to the contact in Outlook, the recurring appointment apperas in the calendar automaticaly. It is sometimes usefull to see, when the appointment has been started.
I have the same problem and still don't have any solution.
