My 4 month old Xperia S (ICS 4.0.4 - 6.1.A.2.45) phone keep crashing and freezing at least 3 or 4 times per day, i did all the repairs with SUS and PCC and nothing, i even send it back to Sony, i waited for my phone around a month and half, and the sad part is that the same problem keep happening, i don't know what do to anymore. Anyone have the same problem? Any solutions?

If I am you I will not install any apps at all and use the phone as it is to see whether it crashes or freezes. If it is stable, than selectively add in apps from Google Play to see which one is causing the problem. Should the phone crash even without any apps added to it, I think your motherboard is gone. If so, Sony should not be returning it to you unrepaired.
As Bearings say, try making another repair of the phone and try it for a few days without restoring any backup or installing any apps and see if it works. If you still have problems with the phone then, try it with another SIM-card and see if the problem remains. If so, the service location must have missed something and i recommend that you contact them again to have this problem fixed.
my xperia was doing this, after some trial and error (and hard resets and reinstalls) i figured it.. i had email setup with the built in xperia app AND with the standalone apps (outlook and gmail) that synced automatically thru google play..
it sounds daft, but when i deactivated the built in app it worked fine, i suppose there was a conflict whenever a mail was recived, it tried (and failed) to split one message two ways and this messed it up, seemingly at random
this probably happens with other apps too, so so sooo id sugest making sure there is only one instance of each app you have running
sometimes its the little obvious things that get right on your **bleep**
