xperia e4 sd card problem

Hi. i had a 128 gb sandisk sd card and insert it in my phone. I used it with no problem for a week. But then i saw the "sd card has been removed" notification. Then i tried sd card on another phones but it didnt work. Can you help me ? Did sd card crash? (Sorry for bad english)

Where did you purchase the card?
Have you checked it using an SD card tester?

My cousin gave it to me. But it was unused, it was in its pocket .No i didnt check. Actually i dont know how to do
