Rare Camera Focus Problem

When I take picture from rare camera, picture become MILD and unfocuse,plz help

Plz note this problem is also occurring in video as well

I've seen this before and the problem was either 

1. a protective case or film too close to the lens
2. dirty lens 
3. scratched lens

there is no problem as mentioned by u
