There are black lines on the images i capture and even before that. Lines run horizontally from left to right. Is this a problem with hardware or software. I even tried running Xperia Diagnostics and images captured there are blurred.

I have already tried resetting the phone, repairing it via Sony Companion, cleared data and restarted the phone.

This can be a software or a hardware issue, You should take your Unit to the nearest Sony Service Centre. They will either repair the software or will apply for a replacement.
