Can you advise of any troubleshooting that you may have gone through since the issue started?

The section that says "Safemode Instructions"
Can you try clearing the Phone app data.
Menu > Settings > Apps > Phone > Clear Data
However it may be a good idea to backup your data using Xperia Companion just in case.
