When did this start happening? Was it after something happened to the phone?
Have you tried several headsets to see if the same thing happens then? I suggest that you start by going to Settings > Apps > ... > Show system > Smart Connect > Storage and press Clear data. Then restart your phone and connect your headset.
If the above didn't help try rebooting your device to safe mode. Should that not help I recommend that you perform a software repair using Xperia Companion.
