issue with camera orientation

Capturing the photograph of a document in portrait mode but it appears in landscape in album. Why the pic is rotated? It should be in portrait if captured in portrait mode.

Settings-apps-all-camera,force stop and clear data. Try it again.

I tried both 16:9 and 4:3.

What if you rotate the device in widescreen mode? I know that way you will see the picture vertically.

Yes Vertical, but sometimes its upside down.

If you capture it in portrait mode,you will get it landscaped. Not sure why sometimes it is upside down. @uliwooly do you have any idea?
