controls its own volume when headphones are plugged in

Hi on top of my many other problems with this god awful phone it has recently started controlling its own volume when head phones are plugged in. when i have headphones on it will turn the volume up and down its self then not recognise the headphones and start controlling the volume of the ringer it will usually either put its self on silent or puut the ringer on full and then the slider will refuse to go away. It also when they are in when i unlock the phone the smart connect app always comes up.
