As far as I am aware, the M5 doesn't have a built in QR/Barcode scanner app. What app are you using for this?

I tried barcode scanner app  , but this issue faced me with the main camera when i want to take a photo for any written screen or documents

No, I haven't screen protector or case, and phone are Close to screen for scan Barcode, and the problem with close shot
