I have been storing all my photos taken with the camera on the SD card.
Today, all the pictures are showing the default 'mountain' icon, and I cannot access the photos on the phone or the PC.
Any ideas how I can restore them?
Try this:
*Remove/insert SD-card
*Insert SD-card in another phone
*Try to install a file manager, can you see your pictures via File manager?
Have you done some type of reset/master reset/format in the phone?

No - doesnt work.
Luckily the photo's have been backed up to Google+ (or Picassa on the phone).
How do I get these back on the phone itself?
have you reformat the SD card?
try this
settings > apps > all > album > clear > restart your phone
Done the above but didn't work - I have now deleted the 'empty' files - as above they are backed up on google+ but I want them back on my phone.

No as it has my music on it. Not sure why I would do that?
install OI file manager > open the app > DCIM > 100Andro > what do you see there?
Its empty - like I said above I have deleted the files in the folder as I couldn't open them.
the only way this could happen is if you reformated the SD card, or if you mounted your sd card to your pc and transfered the pictures and then delete them, PCs could do this delete the pictures it transfer
