You could try to delete previous dhcp leases from your router.
Having the same SSID too tends to trick things, sometimes.
i have a similar issue with our MacBooks: they seldom connect to 5ghz (since a couple of updates) where iPads, other computer and my Z3 compact never miss.
The culprit, I discovered, it's the router's firmware.
Probably you already tried but it's worth to ask;
1) Forget and reconfigure network on your phone;
2) Check the channel of your wireless (but if you have thick bricks other's routers should not interfere);
3) Check the country of your router with some sniffer because it may deal with channel;
4) split SSID to see what happens;
5) Check the firmware of your router to see if there is an update;
6) Try with another router if you have it, or if a friend has one to borrow.
My case.
My own router E4200v1 and macbooks seldom clash since a couple of OS X versions, where z3, iPads and other stuff do not. Problems occurs during negotiations because E4200 sports an outdated firmware and our macs seems to be picky in that.
iPads seems to not care so, I guess, it's a burden between router's and Macs' wifi chip, respective firmwares, and negotiation code.
My ISP branded (and brand new) router does not have problems in this way and macs are happy but, because it's functionalities are trivial at best, I prefer to use my old router.
One day or another I will take a shot and install dd-wrt or openwrt firmware (probably dd-wrt) and see if my E4200 behaves. Maybe you could consider to do the same. Besides, if you have thick walls, consider that dd-wrt lets you choose the signal power beyond the cautionary specifications of the manufacturer.

Thanks for the tips. Yes, I'd tried most of that. There aren't many 5GHz networks near me (only one), and it's pretty far away, channel wise. I'm also pretty sure it's a phone change, as it started happening in December (my Z3 updated in december also) and I never had any problems before then. Currently, I've just moved the 5GHz signal onto a different SSID and not connected my phone to it. I can add any other devices that support 5GHz to that SSID, but I liked the transparency of having them both on the same SSID so the devices could make the decision.
