If the ear speaker test failed while the phone was rebooted in safe mode I would recommend that you perform a software repair using Xperia Companion to rule out that this is caused by the software in your phone. Do note that this will delete all data that is stored on your internal storage so I'd recommend that you backup using Xperia Companion.

When you are using the speaker function during calls the phone is using the second microphone and not the main microphone, so in your case the troubles sounds like they may be related to main microphone.
I would recommend that you start with rebooting your phone in safe mode and either run the diagnostics test for the microphone or that you place a few calls and that you aren't using the speaker function on your phone.
There is a few action you can take to free up space in your phone.
First of all, go to Settings > Storage, how much memory is being used by cached data? This is temporary data for all applications in your phone and in most cases have no impact on the app itself since the data can be downloaded again. If you are OK with deleting this data, tap on "Cached data" and tap on OK.
If you aren't using all the apps in your phone and they can't be uninstalled you can instead uninstall updates for them, this will downgrade the app back to factory settings which is smaller in size than the fully updated app. Go to Settings > Apps and check in the list here if you can find app(s) that you aren't using > tap on one of them > 3 dots > Uninstall updates.
If you have set the Google Play store to automatically update your apps you will have to turn that option off OR search for the app you uninstalled updates for the Google Play store > 3 dots in the top right corner > uncheck the box next to "Auto-update". To turn off the option that apps should be updated automatically, open up Google Play store > 3 lines in the top left corner > Settings > Auto-update apps > Do not auto-update apps.

I'm sorry to hear that. 
I just want to make sure that I understand you correctly, when you make or receive a call you can't hear the counterpart, however if you have a headset connected or use the loud speaker you are able to hear the other person you having the call with, is that correct?
If that is the case, I'd recommend that you test the ear speaker in your phone, this is the speaker that you hear the voice of the counterpart when a call is active and loud speaker is not activated or a headset is plugged in.
You can test the ear speaker in the inbuilt diagnostics test in the phone which you will find in Settings > About phone > Diagnostics > Test > Ear speaker, put the phone at your ear and listen, can you hear the tone being played?

Does this occur in every call that you make or receive? If it does, please try the ear speaker in your phone in the. You can do so in the inbuilt diagnostics test in the phone which you can find in Settings > About phone > Diagnostics > Test > Ear speaker.
If you have the possiblity to connect a headset to your phone and make a call, is the counterparts voice blurred when using headset as well?

Hi,I cannot hear people talking on my sony experia aqua unless I switch to speakers.I tried safe mode to no avail and am getting niggled . Is anyone familiar with this problem or do I send it for repair?
