The music volume (Media volume) and in-call loudspeaker (In-Call volume) have different volume settings (sliders) in the phone menu.

In the first instance I'd advise making a call, putting the phone on speaker, and then toggling up the volume key to make sure In-Call volume shows as maximum. This is also independant of In-Call volume when listening through the standard earpiece during calls so just because your In-Call volume on the earpiece is max doesn't mean it is on the loudspeaker.
